If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery. Find out which GPs you can register with on the NHS website.
You can inform us of a change of address, phone number or e-mail by completing a change of contact details form online or at the surgery.
Non-urgent advice: Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate. Please bring this to surgery along with a change of contact details form
