When does SSP apply?

The government is legislating for SSP to be paid from day 1, rather than day 4, of your absence from workif you are absent from work due to sickness or need to self-isolate caused by COVID-19. Once the legislation has been passed, this will apply retrospectively from 13 March. You should talk to your employer if you are eligible for SSP and need to claim.

Do I need a sick note?

From Friday 20 March onwards, those who have COVID-19 or are advised to self-isolate will be able to obtain an “isolation note” by visiting NHS 111 online and completing an online form, rather than visiting a doctor. For COVID-19 cases this replaces the usual need to provide a “fit note” after seven days of sickness absence. Isolation notes will also be accepted by Jobcentre Plus as evidence of your inability to attend.

New Patient Registration

Patients wishing to register at the practice are required to collect and complete two forms available at reception.

Form 1 is a registration form.

Form 2 is for you to provide any relevant medical history our general practitioners may need until your medical records are received from your previous general practitioner.

We require proof of your present address, e.g. a current utility bill at the time of registration, and if you are newly arrived in this country we will also require proof of identification, i.e. your passport or visa.

Once these forms are completed and returned, you will be asked to contact the surgery within 48 hours to check on your acceptance. If you are successful, you will need to arrange an appointment for a New Patient Check to be carried out with one of our practice nurses.

It is also important that we are provided with your current contact number.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
